This form is for Affiliate Coordinators and Chaos Coordinators who need to make a leadership change - whether that means stepping away, switching roles, or requesting a deactivation. If you’re not ready to step away completely, you also have the option to request a temporary hold of up to 3 months, which allows your affiliate to pause while you decide on next steps.
